DescriptionDevelops, implements and maintains a comprehensive program to correspond with the CMS requirements for the Comprehensive Joint Replacement Program. Follows patients through the continuum of care and establishes relationships with healthcare organizations and providers in an effort to ensure patients are provided with the highest quality of care for the lowest cost. Provides reports, data and decision making information to the B&J co-management company as requested.Qualifications Bachelor's degree in social work, health care, business or related area. Bachelor's degree in unrelated area will be accepted with four years of hospital experience. Electronic medical record experience also required. Prior healthcare experience required, with business development, intake or case management experience preferred.
