Description Role Purpose: Under general supervision, performs selected occupational therapy procedures and related tasks. Responsibilities: Assists in formulating individualized treatment plan for each patient approved by a certified/licensed occupational therapist based on evaluation and information gathered from Social Services, physician, patient and his/her family. Schedules patients for treatment in accordance with physician, patient assessment and treatment plan and conducts treatment sessions, individually or in a group, under general supervision of a certified/licensed occupational therapist. Maintains accurate and timely records of all treatment procedures, including progress notes, discharge summaries and daily documentation as approved by certified/licensed occupational therapist. Continually evaluates patient progress and makes necessary revisions to treatment plan in order to meet the patient's needs and treatment team goals. Consults with and supports treatment plans of physician, patient, family and other allied health professionals. Provides education and training to patients and family members. Assists in the training and supervision of clinical students.
