The Case Management Associate supports the Case Manager in coordinating, procuring and managing resources during the implementation of the discharge plan of complex patients in order to facilitate quality care, improve patient cost outcomes and assist in assuring a smooth transition home or to the next level of care. The Associate educates patients, families and staff on the benefits and availability of home health care, hospice, skilled nursing facilities, long term care facilities, rehabilitation services and home infusion services. The Associate is knowledgeable of insured benefits regarding medication as well as community resources available to the uninsured and assures that patients have the means to obtain their discharge medications.
Bachelors degree in any field. In lieu of degree will accept 4 years customer service experience or experience in the insurance field/social work/medical fields.
Two years customer service experience or experience in the insurance field/social work/medical fields
Basic Life Support (BLS) training or retraining is required during orientation prior to providing patient care. Current Basic Life Support (BLS) is required to be maintained thru re-training for duration of employment.
Basic computer skills. Excellent communication both oral and written. Highly organized and self motivated.
