Job Description

Role Purpose:

Interview patients and family members referred to the department by their family physician and obtain information about home environment, family relationships, health history, and patient's personality traits. Counsel patients and family members through individual conferences to understand, accept, and follow recommendations given by Case Manager or their physician. Assist family and patient in consult with other medical team to promote discharge to appropriate level of care by communication to alleviate apprehension through promote and ongoing communication. Utilize such resources as family and community assistance agencies to aid patients in resuming a productive life within the community. Assist with placement of children in foster homes and/or their families during illness.

Employees in this position are, or can be, required to possess the following physical capabilities:

  • Lifting, carrying, pushing and/or pulling up to 10 lbs.
  • Stooping, kneeling and/or crouching daily
  • Standing or walking more than 5 hours a day
  • Reaching, grasping, fingering, and/or feeling more than 5 hours a day.
  • Visual acuity in order to correctly distinguish skin tone variations, evidence of IV infiltration, infection; able to read telemetry, blood pressure gauges, etc.
  • Audio acuity to discern sounds made by various types of machinery or equipment that would indicate dangerous or improper operation
