Responsible for making face to face patient visits monthly on patients in palliative care program.

POSITION PURPOSE:

Responsible for making face to face patient visits monthly on patients in palliative care program.

Qualifications

POSITION REQUIREMENTS:

Licenses or Certifications: ARNP required

Total Education, Vocational Training, and Experience: Graduation from an accredited program, completion of 675 clinical hours and Masters in Practice Nursing.

SKILLS AND ABILITIES: Ability to seek assistance as needed. Ability to adapt to changes necessitated by patient/staff situations. Ability to write legibly; Ability to interact in a positive manner and communicate well with physicians, patients, families, co-workers, supervisor and other employees. Ability to initiate schedule and oversee caseload.

PROTECTED HEALTH INFORMATION:

Position has Full Access to Protected Health Information of patients on caseload.

Proficiency or Productivity Standards:

Organize and prioritize tasks and projects

Accurately complete job responsibilities within the specified time constraints to meet the needs of patients, regulatory standards and customer satisfaction

Physical ReQUIREMENTS:

ESSENTIAL FUNCTIONS:

Ability to speak, hear, and read English

Ability to perform computer skills

Ability to discern temperature, texture, size, and shape

Exceptional attendance is a requirement in order to fulfill role expectations

CRITICAL DEMANDS:

Lift up to 50# from the floor to 36(Occasional) Stand(Frequent)

Sustained forward bending(Occasional) Sit(Frequent)

Front Carry with 25# for 15 feet(Occasional) Climb stairs(Occasional)

Push up to 75# of force at 36(Occasional) Sustained Squat(Occasional)

Full Body range of motion(Constant) Repetitive Squat(Occasional)

Grasp of various sizes and weights up to 50#(Constant) Kneeling(Occasional)

Balance(Continuous) In walking, standing, carrying, moving patients, and equipment, etc.

ENVIRONMENTAL CONDITIONS

Indoor climate-controlled environment. Patient care environment with potential exposure to unpleasant odors, to blood and body fluids which may carry infection to infectious disease, and to chemical and electrical hazards. Daily travel with exposure to outdoors elements. Must have reliable transportation and maintain adequate automobile insurance coverage.

Equipment used for patient care. Office equipment such as computer, FAX, telephone, copier, calculator, etc.. Assumes responsibility for equipment, inventory, safety, and security of assigned facilities. Equipment and keys checked out to employee is to be kept in working order and must report lost, broken, or stolen equipment immediately.
