Job Description

The position serves as a primary source to coordinate and communicate the financial management, interdisciplinary clinical service delivery and discharge planning for PACE participants throughout their enrollment.

Qualifications
POSITION REQUIREMENTS:

Licenses or Certifications: LSW, CPR Certification.

Total Education, Vocational Training, and Experience Masters degree in Social Services, at least one (1) year experience working with the frail or elderly population.

Skills and Abilities:

Working knowledge of regulations and parameters of Medicare/Medicaid reimbursement;

Exhibits tact, diplomacy, and team-player attitude working toward customer and hospital-wide objectives;

Protects confidentiality in all areas as applicable;

Ability to dictate patient documentation;

Ability to demonstrate skill in administration, accountability, decision-making, and time management;

Ability to interact in a positive manner and communicate well with patients, families, co-workers, supervisor, and other PACE employees;

PROTECTED HEALTH INFORMATION:

Position has Full Access to Protected Health Information of patients on Rehabilitation Unit and in Outpatient.

Accurately complete job responsibilities within the specified time constraints to meet the needs of patients, regulatory standards and customer satisfaction

Ability to speak, hear, and read English.
